In order to apply for the Work-Based Learning program, see the requirements below that you must meet. Please also review the application process, so that you'll understand what to expect.   • Have an identified career goal

  • Must be a junior or senior & at least 16 years old

  • Provide your own dependable transportation to/from job site

  • Maintain good attendance record

  • Be willing to take high school and technical/college courses related to future employment within the identified career area

  • Submit at least two teacher recommendation forms

  • Be on track for graduation

  • Willing to submit to all health related screenings required by the sponsoring employer

In addition to meeting the WBL program requirements, students interested in working at the hospital must also complete additional documentation due to the work environment that you will be placed in. The application process for hospital internships is also different, so please note the documents and dates below.

Students must complete the WBL Application, Teacher Recommendation forms, WBL Documentation Packet, and work permit (if under age 18) due to your WBL Coordinator, PLUS the hospital system documents, which are listed below and can be found to the right:​

  • Confidentiality and Security Agreement Form signed (refer to CD)

  • Information Zone Certificate (CD)

  • Copy of Program Application & Teacher Recommendations (minimum of 2)

  • Copy of the one page essay (Why do you want to be a part of the Northeast Georgia Health System Internship Program?  - should be typed; 12 point font; double spaced with heading) – be sure to include how this experience relates to your future career goal

  • Patient Privacy and HIPAA Certificate (CD)

  • Corporate Compliance Certificate (CD)

  • NGHS Compliance Handbook Acknowledgement (refer to CD)

  • Copy of Youth Apprenticeship Conduct Contract

  • Job Shadow/Observer Acknowledgement signed

  • Rotation Times/Carpool requests

  • Medical tests (listed below). You must have these tests/shots                                                   completed with your own doctor. 

    • TB skin test (within the past year)

    • MMR (a series of 2)

    • Hepatitis B Vaccine (at least one of the series of three)

Mandatory Orientation: August 13, 2019 at 5:30 pm.
